If you have recently moved to the district, we will need the following information:
- Certified copy of birth certificate for all students
- Proof of address
- Copy of one of the following at address within the district
- Real Estate Contract
- Lease Agreement
- Rent Receipt for the latest month
- Or one of the following showing address within district in parent's name
- Most recent utility bill (Gas, Electric, Water) NO PHONE BILLS
- Deposit receipt for Gas, Electric, or Water
- Last report card if applicable
Please bring this information with you when you enroll your student(s) along with your student's demographic form. Only one
demographic form is needed per househould. If a student lives at multiple addresses a second form will be needed. Please
list everyone who lives in the household including minors who do not attend USD 437 and all adults who live at the address.
If you do not live within the USD 437 school district, but would like to request permission for your student(s) to
attend a USD 437 school you will need to complete the
Out of District Request Form. The Board of
Education
MUST approve your request before you can enroll your students.
